                                            BANK DRAFT/ACH DEBIT/ELECTRONIC CHECK (PAYMENT) AUTHORIZATION

                                            To ensure the correct account number is used for this electronic recurring payment authorization and to obtain the ABA/Routing number PLEASE CONTACT YOUR FINANCIAL INSTITUTION.

By providing the above information you agree that Daystar Television Network has the right to perform pursuant to an I agree to be bound, as if Daystar Television Network and Depositor each entered into the following terms and conditions individually in writing:

1. If at any time a draft drawn, ACH debit or electronic check transmitted by Daystar Television Network under this arrangement against the designated account is not honored in the usual course of collection and the amount thereof unconditionally credited to Daystar Television Network, this Agreement shall terminate automatically at the sole discretion of Daystar Television Network. Termination may also be accomplished as follows:

(a) by you with written notice to become effective 30 days after receipt thereof at Daystar Television Network offices in Colleyville, Texas, or 

(b) by you to your bank at least 3 days prior to the scheduled date of payment, or

(c) by Daystar Television Network to become effective 30 days after notice of termination is mailed to your last known address, or

(d) by the bank to become effective 30 days after notice of termination is by Daystar Television Network at its offices in Colleyville, Texas.

2. YOUR REGISTRATION AND THIS AUTHORIZATION SHALL BE EFFECTIVE UPON YOUR RECEIPT OF AN E-MAIL CONFIRMATION FROM DAYSTAR TELEVISION NETWORK, WHICH IF NOT CANCELLED BY YOU WITHIN 72 HOURS BY RETURN E-MAIL, WILL BE YOUR FURTHER AND ADDITIONAL AUTHORIZATION FOR DAYSTAR TELEVISION NETWORK TO DRAFT, ACH DEBIT OR ELECTRONIC CHECK YOUR ACCOUNT ON A RECURRING BASIS. THE DEPOSIT BY AMERIPLAN OF THE FIRST DRAFT, ACH DEBIT OR ELECTRONIC CHECK TRANSMITTED UNDER THIS AGREEMENT SHALL CONSTITUTE ITS ACCEPTANCE OF THIS AGREEMENT.

3. Daystar Television Network shall incur no liability if the balance in your account is insufficient to cover any draft, ACH Debit or electronic check upon presentation.

4. The cancelled draft, bank debit memo, or monthly account statement will constitute your receipt for payment of your pledge.

5. You agree that Daystar Television Network may make any appropriate debiting or crediting transactions as needed to correct debits made in error.

6. You hereby direct Daystar Television Network to draft, ACH debit or electronic check your account on your pre-selected day of the month.

You hereby authorize Daystar Television Network to draw drafts, ACH debits or electronic checks from time to time on the account identified by you at your bank for payment to Daystar Television Network in the amount pledged. Unless terminated by Daystar Television Network, you or bank as stated above, this authorization is to remain in full force and effect until Daystar Television Network or the bank has received written notification from you of its termination in such time and in such manner as to afford Daystar Television Network or the bank a reasonable opportunity to act on it, or the pledge amount has been fulfilled. 

You are urged to print a copy of this page for your records. If you desire, a copy of this authorization will be sent to you upon written request.
